She is currently the Three County Continuum of Care, Coordinated Entry Specialist with Community Action Pioneer Valley who has worked within our communities in many roles over the last 14 years.

Mike Fritz Community Builder Award

The Communities That Care Coalition’s Mike Fritz Community Builder Award was created to honor Mike Fritz, the local business owner whose vision and generosity helped to found the Communities That Care Coalition in 2002.  The award is given each year to a community member who demonstrates vision and/or leadership in integrating the programs, policies, practices or goals of the Communities That Care Coalition into their own agency, institution, or work for the betterment of the community.

Sara Cummings Coalition Leadership Award

The Sara Cummings Award for Coalition Leadership was created to honor Sara Cummings, former CTC Coalition Co-Chair, for her work during the first 6 years of the Communities That Care Coalition. It is given each year to a Coalition member who demonstrates leadership, inspiration, and/or advocacy for the work of the Communities That Care Coalition.

The 2022 award will be presented to Shaundell Diaz of Community Action Pioneer Valley’s Three County Continuum of Care. Join us on June 9th from 3:30-5:00 at Greenfield High School’s Cafeteria for this coalition meeting!
